Forms no longer render properly

I use MailChimp exclusively and place their sign-up forms on multiple websites. On the iPad it works just fine in FF.

But suddenly after the latest browser updates, MailChimp form code is rendering like HTML 1.0 and not picking up the formatting even when I use a plugin for embedding code.

Below are screen shots of what it looks like on dozens of sites and what it SHOULD look like, taken from my iPad which still works OK (winnigop.org).

MailChimp claims they did not change anything so I have to assume it's the browser.

How to fix?

Sorry about that, the second photo of how it is supposed to look did not post. Trying with a smaller version.
