How can I have my business website/online portfolio unblocked if it was mistakenly thought to have adult content in it?
I am a professional photographer. Once upon a time, I shot adult content. However, I switched directions, I currently shoot fashion, and I haven't shot erotica in over five years. My currently website is clean and workplace-friendly. It has come to my attention that many offices, including those of clients/potential clients, are unable to view my site because of adult content restrictions. I fear I am losing business as a result. Is there a way to get the adult content label lifted from my site?

You have to reach out to the content filter makers who run these lists that are blocking you.
Is there a way to determine which content filter makers have blocked me, or do I just "spray and pray"? (Contact a bunch and hope it works)
Thanks for your reply!
IMO, it would be easier to get a new domain name for your new direction and leave your past in the past.
There is good chance that your old content is archived somewhere (like in the Wayback Machine - http://en.wikipedia.org/wiki/Wayback_Machine ) and that old content might still be available via internet searches for your website.
Do you know whether it is blocked by domain name or by IP address?
If it's by domain name, It's not very expensive to register a second domain name and alias it to the same root folder as your current website. Unfortunately, Google is allergic to "duplicate content" so you might need to hide the second domain from search engines and just give it out to clients who have problems with the main domain, or routinely give out both domains. This could be a short term workaround until you get the lists updated.
If it's by IP address, you would need to change your hosting to escape the block, which your host might or might not be willing to help with.
