I am looking for a way to set custom icons for the items on my bookmark toolbar. I have quite a few that end up having the 'generic' internet globe and, since I have the titles turned off, being able to customize the icon would be very helpful.

The same applies to Bookmark folders. They all appear as just the folder icon. It would be nicer if we could set a different image to tell them apart.

Hmm. I have a number of Tabs with the 'Globe' icons, mixed with my other Tabs with 'proper' icons. I think it started to happen when my main browser window grew in size with many Tabs over time (over 1,500 now).

Typically the web site icon (Favicon) isn't saved immediately when saving a new bookmark, but rather the icon gets saved when the bookmark is used for the first time.

Bookmark folders are all the same. There is no add-on to add a feature to Firefox which would allow the user to have customized folders. But that is possible via userChrome.css, but that isn't exactly an easy modification for most users.
