Are you using any password add-ons/programs?

Start Firefox in Safe Mode {web link}

A small dialog should appear. Click Start In Safe Mode (not Refresh). Is the problem still there?

If "Ask to save logins and passwords for websites" is checked and stays checked, and you don't use an external password manager, then there are a few possible causes - would you please read everything in this article :
