You are probably running Firefox directly from the downloaded firefox.dmg file. This is a common mistake that many Mac users make and it can cause this type of issue because every time you open Firefox it's treated as a new installation because of how Mac handles .dmg virtual disks.

You need to open the firefox.app file that's located within the firefox.dmg file to your Applications file. See How to download and install Firefox on Mac.
