I have run Malwarebytes looking for malware, checked for suspicious extensions and add-ins, and even deleted Firefox and reinstalled it from a download. Yahoo remains the default and only search engine.

The official Firefox builds from *.mozilla.org has not been bundled with Yahoo in any locals since Fx 57.0 on, more so now because of the lawsuit. [forums/contributors/712925]

A few possibilities. You downloaded and installed a version of Firefox not from mozilla that came with only Yahoo search engine. An Extension that was installed or updated recently changed and or added the Yahoo search engine. A external software added the Yahoo search engine.
