I see the Back and Forward button at the far right on the location/address bar and the refresh button is missing. In Firefox 57 these items are customizable, so you may have (re)moved the combined Stop/Reload button as well that would normally appear at the far left of the Navigation Toolbar next to the B/F buttons.

Make sure toolbars like the "Bookmarks Toolbar" are visible. Open the Customize window and set what toolbars (Show/Hide Toolbars) and toolbar items to display.

  • Right-click empty toolbar area -> Customize
  • "3-bar" menu button -> Customize
  • View -> Toolbars
    *you can tap the Alt key or press the F10 key to show the hidden Menu Bar temporarily
  • check that "Bookmarks Toolbar Items" is on the Bookmarks Toolbar
  • if "Bookmarks Toolbar Items" is not on the Bookmarks Toolbar then drag it back from the Customize palette to the Customize window to the Bookmarks Toolbar
  • if missing items are in the Customize palette then drag them back from the Customize window to the toolbar
  • if you do not see an item on a toolbar and in the Customize palette then click the Restore Defaults button in the Customize palette window to restore the default toolbar set.

cor-el's reply covers the logistics of rearranging the bar, which you already know about. But what happened here?!

Normally a minor update like Firefox 57.0.x to 57.0.4 would not relocate those buttons. Very strange!

The button order/selection is stored in your prefs.js file (and you can see it in about:config) under the preference

browser.uiCustomization.state

Did you notice any other unexpected settings changes that could indicate that file got corrupted somehow?

That sounds like a problem with the prefs.js file (browser.uiCustomization.state) if changes aren't kept. You can check prefs.js in the profile folder of the Firefox portable version (it in is its installation folder) to see if it in modified.
