address won't populate when creating a new tab or a new window. back & forward, favicon and refresh disabled. What do i do to get these back?

Firefox 6. When opening a new tab or new window, back and forward arrows, favicon and refresh are disabled. Url field will not populate with current window, but if I click on the favicon the url will populate with the current web site. At the same time a message is display "this web site does not supply identity information".

I disabled all plug-ins and extensions. the problem has been resolved. Now I will enable each one individually to find out which one was the offender.
