The LastPass icon has disappeared from the menu. I've tried re-installing the extension, and looking in the "customize" list (right click on the menu area), but it isn't there. Please advise how I can get it back. I rely on it. I think this is a result of a Firefox upgrade.

Have you looked in the extension panel with the jigsaw puzzle icon?

If you are using private windows or permanent private mode (aka "Never remember history"), you will need to give the extension permission to run.

Right-click the extension on the extensions button or click the cog alongside the extension on the button then Pin to Toolbar.
