I solved it! I was using Keyboard Maestro to launch FF with a keyboard shortcut of: Shift-Control-Option-Command. It must have been the "option" key in that shortcut.

Normally you would only get that prompt if you held down the Option/Alt key when launching Firefox. ??

Any chance that you accidentally set that as "your Homepage" in Firefox?

Normally Firefox should only start in Safe Mode when only the Option key is held down and not when additionally other modifier keys are pressed like in your case Shift and Control. If that is happening then this sounds like a regression although I don't know what Firefox sees if you use four modifier keys and not a real key to launch Firefox via this utility.
