One of my toolbar bookmarks is "null" and can't be accessed or removed, any fixes?

The bookmark toolbar only displays the bookmarks to the "left" of the null bookmark. When I open the bookmark list, the offending link is still visible but when highlighted it doesn't show any url.

This can be a problem with the file places.sqlite that stores the bookmarks and the history.
