Or is the problem the change to the compact menu (orange Firefox button) in Firefox 20? In Firefox 20, New window is now a sub-choice under New Tab as shown in the attached screen shot. Since I always press Ctrl+n, I hadn't noticed that until just now.

Edit: For the record, that's actually Firefox 21 in the screen shot.
