why does open a window tab not work

When you click on the open a new window tab nothing happens. However when you choose new window from the File drop down menu it does work.

This happened

Every time Firefox opened

I forgot to say that if you click the open new window button when you close Firefox it will not restart until you close it through the task manager processes window.
