firefox doesn't auto fill in passwords

Notice a lot of the times firefox will either (1) fill in the password but not my user name (2) fill in my username but not my password (3) not fill in either

It's saved to my firefox, I can go to my password lists and see them, but it won't fill it in once I'm on the sites. (and yes, I am visiting the correct website not a variation of the site like gmail.com and not gmail.ca

One user had this issue. Turns out the 'fill in login' was turned off.

Does this happen on the same websites, different sites, sometimes it works sometimes not . . . . .
