PDFs opening twice using 'open with > Firefox' when browser is not already running

Windows 7 - Firefox 15*-16*

if Firefox is not already running, opening a PDF document using Firefox (eg right-clicking PDF from desktop, selecting 'open with > Firefox) is always causing Firefox top open the PDF in two different windows. Happening with PDF plugins enabled and disabled.

Maybe this is a bug and I should report as such?
