Since I upgraded to Firefox 4, the "New Tab" button sometimes appears close to the rightmost tab and sometimes is pinned to the right side of the tab bar. I grabbed a couple of screen shots that illustrates it:

That makes it quite annoying to open a tab with the mouse since I first need to look for the button.

Is it a known issue?

That new tab button moves to that position if there are so many tabs open the the tab scroll buttons appear. Your screenshot shows those scroll buttons, but that shouldn't happen with only one tab open and lots of space left. You can check if your issue is caused by an extension or theme that isn't working properly.

Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the add-ons is causing the problem (switch to the DEFAULT theme: Tools > Add-ons > Appearance/Themes).

  • Don't make any changes on the Safe mode start window.

For the records: a silly workaround is to open a new window (Ctrl+N) and drag there the tags from the old one.
