Pop up window upon FireFox launch asks do you want to review FireFox and we'll give you a small gift. Answer 4 or 5 questions involing use of firefox and it leads to a sales pitch/trial web site. Is this a Mozilla add-on or a scam?

hello, unfortunately you are right to be suspicious about that. this appears to be a scam tactic and no genuine messages by firefox or mozilla, since we don't run such kind of business or promotions, we are focusing on developing great software products instead!
maybe it's a problem caused by adware on your pc. please go to tools > addons > extensions & remove any suspicious entries (toolbars, things that you have not installed intentionally, don't know what purpose they serve, etc).
Remove a toolbar that has taken over your Firefox search or home page Troubleshoot Firefox issues caused by malware
if it is a website that is using the firefox brand in order to attract attention or misleading users to participate in these surveys, you can report that as a trademark violation to mozilla at https://www.mozilla.org/en-US/legal/fraud-report/ so that further action can be taken on it.

Yes, thanks, I keep forgetting to check the add-ons/plug-ins. I deactivated a couple, including SilverLight (a known issue) and it appears to have stopped. Thanks!
