Folders suddenly become subfolders unbidden - how to reverse this and stop it from happening in the future?!

Without my doing anything (that I'm aware of) a main folder will suddenly become a subfolder to another main folder (what I'm calling "main folders" are all in the drop-down list called "Local Folders").

I now even have s subfolder that is part of another subfolder -- all done spontaneously and without any apparent action on my part! It makes finding data tricky because I have to remember this now unwanted folder structure, and sometimes have to go on a hunt for files.

What I'm looking for are the steps to take to return all these subfolders to the main folder or "Local Folder" position, so I can see them all when I open up the "Local Folders" drop-down menu.

Could you provide a screen shot demonstrating this? I wonder if you're using any characters that have special significance in filenames.
