I want to have a button on the addon tool bar that will enable and disable flash player. When I try to customize the addon bar, there is no option for flash.

Probably be easier if you just added a shortcut for your main plug-in/add-on page rather than flash specifically. You'll need to enable to add-on bar as its off by default unless you have already turned it on, right click anywhere in the tab strip and select it from the pop up menu.
