Why are all my pop-up windows never on top anymore?

Pop-up windows on websites used to always open on top. Now they ALWAYS open underneath the web page. This makes them useless. I'm not talking about ads. This issue apparently has nothing to do with any add-ons. If there is a FF setting to change this behaviour please let me know. After the painful process of posting this issue, if you can not resolve please say so, and I will simply choose another browser.

This can be caused by a plugin-container process that runs in the main window.
