Often when I middle click on a link, the new tab that is opened is not the link I clicked, but the default new tab. If I middle click the link a second time, I end up at the correct site. What should I check?

Try disabling that Sogame NewTabURL 2.2.3.1 extension. It's old (Released August 13, 2011) and the latest UserReviews say it's broken. https://addons.mozilla.org/en-us/firefox/addon/newtaburl/reviews/
