Missing "Page actions" and/or Bookmark Star In Address Bar
Problem: I had inadvertently removed my bookmark webpage star from my browser (Firefox v.70). I searched and couldn't find an answer as to how to get it back on my address bar. The closest answer involved clicking the "Page actions" (which looks like an ellipses; 3 dots) button which is adjacent to the bookmark webpage star button by default. Both were missing from my address bar.
So I refreshed the browser and had to manually adjust all my settings again and reload my 2 extensions however neither the "Page actions" button nor the bookmark webpage star button were present after the refresh.
Solution: Refresh the browser. Right-click on an empty area of whatever webpage. A context menu will popup with a star that appears in the upper right side of that popup window. Make your new bookmark and click "Done". When I did this both my "Page actions" and "Bookmark Star" were back in my address bar.
I hope this helps people like me who find either or both of those missing.

You should normally have the page actions button visible if you have opened a web page in a tab. You only see the page actions icon if you are on a web page where this applies (i.e. it is missing on a blank tab and possibly on some about or error pages). In some cases like when you restore pages on startup you may have to reload (refresh) the web page).
