Chosen solution
This issue can be caused by the Ask.com toolbar (Tools > Add-ons > Extensions)
Your system details show that toolbar in the user agent as AskTbTRL2/3.12.5.17640
Start Firefox in Diagnose Firefox issues using Troubleshoot Mode to check if one of the extensions is causing the problem (switch to the DEFAULT theme: Firefox (Tools) > Add-ons > Appearance/Themes).
- Don't make any changes on the Safe mode start window.

since I wasn't interested in the Ask toolbar I simply uninstalled it (without going through all the testing) and things work fine now. Thank you.
