Cannot create folder in bookmark tbar since update 45.0.2

Since upgrading to Firefox 45.0.2 I can no longer add folders to the bookmark bar. If I right-click and choose "New Folder," I get the add folder dialog without a textbox in which to type a folder name. Even if I click "Ok," I do not get an empty folder to which I could add a name.

You can check for problems with the places.sqlite database (bookmarks and history) in the Firefox profile folder.
