Without seeing the actual crash report this is just a guess. The fpserviceprovider.dll file is part of the FoxyProxy Video Utility, the crash may be caused by that utility. You can try uninstalling it to see if it stops the crashes.

For info fpserviceprovider.dll@0x1b40 is not the crash report ID, the crash report ID is a long series of letters and numbers similar to this:

12da013b-76ca-43bb-aa90-bc6cf2100428

Quite a few of those crash reports relate to bug 591358

If I am reading the crash report correctly, the cause of the crash has been identified and the fix will be included in Firefox 5 which is due for release on June 21st. There is a query in it about Firefox 4 but that has not yet been answered.
