I almost always have multiple tabs open in firefox and intermittently when I move back to a tabbed window rather than opening that window in my current browser firefox will launch a new (second) browser and open my old tab here - closing down the tab in the first browser. I move between tabs by clicking on the tab and most of the time it will open the tab but doing exactly the same thing will intermittently launch this second browser. All other functions with firefox are operating normally

Firefox 3.5 versions and later have a feature called tear-off tabs.
You can detach a tab from the current window and open it in a new window by dragging a tab in the browser window.
You can drag that tab back to the tab bar in the original window to undo that detaching.
