How Do I get FF 3.6.6 to ask about cookies
I cannot find how to set Firefox 3.6.6 to ask what I want to happen to cookies when any site tries to give me one. Have I simply forgotten where this setting is?

I have the same problem with my Mac ( Intel, Snow Leopard). Keep getting tracking cookies on my machine. Where does Firefox give a Cookies option in its preferences?
Earlier versions allow the user to be prompted when a cookie is being offered. One could choose to Allow, Allow for Session Only, or Deny. The current version does not seem to offer that option. I am disappointed that this feature seems to have been curtailed.
Did you miss Tools > Options > Privacy > Cookies: "ask me every time" ?

There is no cookie option showing after I select "Use custom settings for history"
It clearly shows in your screenshot: "Accept cookies from sites" - "Exceptions" [[ ] "Accept third-party cookies" "Keep until": "they expire"
You need to click the arrow next to "they expire" and choose "ask me every time" from the drop down list

On a Mac look in Firefox > Preferences > Privacy > Cookies: "ask me every time"

I'm using version 3.6.17
I don’t need a choice about KEEPING cookies, I need an “ask me every time” choice about ACCEPTING them - First-party and Third-party. This option existed previously, why was it removed? IE8 has it.
The only choices found here are always or never, even if you add exceptions they still define accepting cookies in terms of always or never.
I realize that the option "ask me every time" whether to allow cookies creates constant confirmation windows popping up which can be very annoying, and I would never use it for general internet use... however it is a very useful debugging tool, and I need a browser that offers it to me. Since you had it before, why not just bring it back?

What is against clicking on "Keep until" and select the "ask me every time" choice?
