what does the menu icon of three lines in a column mean?
I want to uninstall a plug-in. directions show to go to [---] menu. these three lines are in a COLUMN, rather than side by side. What does that mean?

That is the "3-bar" Firefox menu button that you can find at the right end of the Navigation Toolbar.

You can only disable a plugin in Firefox by setting it to "Never Activate" on the "Firefox/Tools > Add-ons > Plugins" page.
- Tap the Alt key or press F10 to show the Menu Bar
To remove a plugin, you would have to uninstall the program that adds this plugin or disable the plugin in that software if possible.
