I installed Firefox for Android specifically because the screen shots show a back button on the screen, however it does not show on my Samsung Galaxy s5. How can I get the back button on the screen? Thanks

Scrolldown / swipe towards the bottom and it should appear. By the default all the screen is dedicated to showing the content, to show the top bar address you need to make it visible on purpose.

The back button is the back button on the phone. Space is limited so that button is not duplicated in the phone UI. On tablets we have a navigation button similar to desktop Firefox.
