I can create a new bookmark, but I cannot create a bookmark folder ... the settings drop down on the "Show All Bookmarks" window is grayed out. Fix?

The menu is grayed out because you cannot create a new folder until you select one of the three bookmark locations (see screenshots).

Once you select Bookmarks Toolbar for example, the drop-down/settings menu will be enabled.
