The menu bar is present, but I can not enter any websites. There is no longer a place to type it in.

I have tried to close and open the menu bar several times (ALT+V T M), and it clearly goes away, but there is still no place to enter any website address. I have also restarted my computer and Firefox.

The Navigation bar is where the Address / Location bar is located (unless you re-arranged your toolbars).

View > Toolbars - Navigation bar

My mistake for calling it the wrong thing. I have all bars selected (Status, Navigation, Menu and Bookmarks). I can still see Go Back and Forward Buttons, the reload button, the stop and home button.

* If on Windows in Firefox 3.6 the menu bar is hidden then press and hold the Alt key down, that should make the "Menu Bar" appear.
* Go to "View > Toolbars" and put a check-mark for "Menu Bar" to make that permanent. See Restore the Menu bar in Firefox).
* Make sure that you have the Navigation Toolbar and other toolbars visible: "View > Toolbars"
* If items are missing then see if you can find them in the "View > Toolbars > Customize" window.
* If you see the item in the Customize window then drag it back from the Customize window to the Navigation toolbar.
* If you do not see that item then click the "Restore Default Set" button in the "View > Toolbars > Customize" window.
