My printing options do not show that. They just say Format For, and Paper size: and Orientation:

This seems to be completely different on each OS, so ignore Windows-specific threads.

This article (switched to Linux) shows an Options tab in the Print dialog (not the Print Setup dialog) where you can set or clear headers and footers:

Yes, this works, I always print from 'preview' so never used the page. Now I'm no longer sure what Page Setup it supposed to be used for, since my print settings are kept in 'print' and the page options are kept in 'print -> options'. Glad to know I have options now!
