Flash player opens as a separate Windows window
Any Flash content opens up flash as a separate window/process outside of Firefox, the Firefox space where it should have rendered remains empty. Flash content is instead rendered in the topleft corner of the computer screen either in front of or behind the Firefox window.
Every flash player will get its own windows taskbar icon which can be closed, when closing it the sound of the clip will keep playing for about 5 seconds before stopping.
I have tried complete uninstall / reinstall of the Flash player plugin. I have tried disabling all other plugins and addons and restarting Firefox. I have tried disabling hardware acceleration in the Flash player plugin.
Flash version is 14.0.0.179 (latest).
There are no problems with the Internet Explorer Flash plugin.

You can try to disable hardware acceleration in Firefox.
- Tools > Options > Advanced > General > Browsing: "Use hardware acceleration when available"
You need to close and restart Firefox after toggling this setting.

You can check for problems with current Shockwave Flash plugin versions and try this:
- check for updates for your graphics drive drivers

- disable hardware acceleration in the Flash plugin

Here are a couple of pictures of the problem. The first shows the empty space in Youtube where the flash player should have rendered. The second shows the desktop when Firefox is minimized and how the flash player renders to the top left corner of the screen and also how the flash player gets its own task bar icon.
This is not specific to Youtube, it happens with all Flash player content on all websites.
I did also reset all the flash player settings in firefox to their default values.
