shift-command-w or command-w does not close the window?

Weird- and this just started... Now when I use command-W or shift-command-W Forefox window won't close. Even if I use mouse and go to menu bar to select close window, Firefox won't close. The only way to close if there is one window open is the red radio button in the upper left corner.

hotkey combination works in other apps no problem.

Evernote Web Clipper is the problem, in my case.

Evernote Web Clipper was the problem for me, too. When I disabled Evernote Web Clipper, Command-W began working as normal again.

This only happens for me on pages where I've allowed some Flash element to play such as a YouTube video (I am using Flashblocker so I have to manually click on Flash elements).

Flash takes "focus" away from the Firefox window and the keyboard then won't work. Click somewhere in Firefox, like the scrollbar to restore "focus" to Firefox.
