everything is now opening as a separate window and not a tab.
Firefox crashed, will only open in safe mode, will not refresh and now all new items that would formerly open in tabs are opening in separate windows. I don't care about recovering what was up at the time of the crash. I just want Firefox to function properly again.

Did you check this setting?
- Tools > Options > Tabs: [X] "Open new windows in a new tab instead"
