Why is it that when I open the "New Tab" page sometimes it shows three rows of four, totaling twelve and sometimes it shows three rows of three totaling nine?

I have twelve sites "pinned" in the "New Tab" window, most of the time all twelve sites are available as there is three rows of four sites but sometimes it switches and only shows nine of my pinned sites and I can't figure out how or why this change is taking place.

This would normally only happen if the browsing area is smaller or if you zoom the page.

You may have accidentally zoomed web page(s). Reset the page zoom on pages that cause problems.

  • View -> Zoom -> Reset (Ctrl+0/Command+0 (zero))

You can zoom out the about:newtab page to resize the tiles and have more tiles fit on the page.

  • View > Zoom > Zoom Out; Ctrl and -
    "Zoom Text Only" shouldn't have a tick

This will affect all elements on the page, so text get smaller as well. Make sure that only one tab with the about:newtab page is open when you change the page zoom. Toggle the browser.newtab.preload pref to false on the about:config page in case of issues with remembering the page zoom of the about:newtab page.
