That's not really a Firefox support question.

Assuming you have a scanner (freestanding or part of an All-in-One printer). Basically, you scan that completed form and save it as an image. Then you send that image by email as an attachment.

You need:

  1. Familiarity with your scanner software.
  2. Familiarity with your email program or webmail software on hoe to send an attachment.
