java and flash were set to ask. changed silverlight from always to ask. exited and restarted ff. didn't fix it. what's next?

starting in safe mode did not fix. disabling extensions stopped it until FF restarted. then it was back. made no difference which extension. plug-ins all set to always ask except "Cisco-Provided Binary of OpenH264 Video Codec" which does not have the option to ask, only always or never. what's next?

Have reset ff and problem persists. I'm not the only one having this problem. what is next???
