Since I upgraded to FF 4, my back and forward buttons don't work, even after trying all of the steps on the FF help page. How can I fix this?

Also, when starting up, FF comes up with the Menu Toolbar disabled, and it looks like there are two FF screens stacked on top of each other. This goes away when I check show Menu Toolbar. None of the steps on the FF help page worked to fix the problem.
